This form is required for any pupils attending an off-site school trip. One form is required per pupil, per trip.

Pupils will be given an EV8 form to sign with the letter informing parents about the trip. The form is included here incase it is mislaid at home.

The first EV8 form is to be printed off and filled in by hand.

The second EV8 form is identical but can be filled in online if you prefer. However it must still be printed and signed for legal reasons.

Only one of these is required and if you have sent the form back your child brought home from school you do not need to use these forms.

The EV8 form replaces the previous OS2 form for off-site visits.
